Create a button to send feedback to the course developers.   A feedback button should be available on every slide.  When the button is pressed a form should appear that allows the user to send their notes about the course.

The first version will submit the comments to an email address.  The comment should include Course Name, Slide Label, Slide Number, Comment, today's date.
